Orléans Family Health Hub



What is the Orléans Family Health Hub?
The Orléans Family Health Hub (OFHH) is a unique concept that integrates a wide range of client-and-family based primary care and community services under one roof.

This innovative project unites major health and community service providers from Ottawa and Orléans, improving coordination of care and reducing the need for clients to transit between various health providers for consultations in their respective offices.

This collaboration will improve the continuum of care for the residents of Orléans.


What are the benefits of this project?
The Orléans Family Health Hub will enable:
- improvements in population health for Orléans and surrounding area;
- better access to primary care by offering a greater range of community-based services;
- the creation of a community-based collaborative care model that improves client access to timely and appropriate care.

In addition, the creation of the Hub will facilitate:
- more cost-effective care by offering solutions close to home;
- an easing of hospital bottlenecks.


Who are the partners involved in this project?
The Orléans Family Health Hub is the product of a collaboration between several of the Ottawa region's health and social service providers:
- The Community Care Access Centre (CCAC);
- The Children's Hospital of Eastern Ontario (CHEO);
- Connexion Family Health Team
- L’Hôpital Montfort;
- The Ottawa Hospital;
- Orleans Urgent Care Clinic;
- Champlain Local Health Integration Network (CLHIN);
- Ottawa Public Health; and
- Bruyère Continuing Care.


Does the moniker 'Mini-Montfort' appropriately describe the Hub project?
No. The OFHH is an initiative of several service providers from Ottawa and Orléans.

The Local Health Integration Network (LHIN) has a mandate to oversee the project planning and to collaborate with its partners to determine what services will be offered at the Hub.

As a key partner, l’Hôpital Montfort is working in close collaboration with the LHIN.


Can the Orléans Family Health Hub be compared to a hospital?
No. For now the Hub will only deliver primary and community care. No hospitalizations are planned for this facility.


What services will be offered at the Orléans Family Health Hub?
The care services provided by the OFHH will be defined in the first planning phases of the project. The Government of Ontario has recently awarded 60 million dollars in financing for this first phase.

The LHIN was given the mandate to lead the project planning.
