STATEMENT OF BIRTH
Ontario residents
Your baby’s birth is registered online by the FBC at Montfort Hospital through ServiceOntario, a new agency for registering newborns living in Ontario.
The service also lets you apply for your child’s birth certificate and social insurance number all at once. Fast and easy!

Quebec residents
You have to get a health-card registration form from the Régie de l’assurance maladie du Québec by calling 1-800-561-9749. Once you’ve received your baby’s birth certificate, complete the registration form and mail it along with the original birth certificate (it will be returned to you). Your baby is covered under your own health card, mother or father, until you receive his or hers.
Birth Confirmation
Montfort Hospital’s health-information disclosure sector can issue you a confirmation of birth. To determine if you need this document or to find out more, feel free to go to Room 00D135 (Level “00”, Wing D) or to call 613-748-4903. Someone will gladly answer your questions and let you know what the fees and the procedure are.
Health Care Team at the Family Birthing Centre
The Family Birthing Centre team consists of registered nurses, registered practical nurses, physicians, midwives, a lactation consultant, respiratory therapists, a social worker, a pharmacist, a nutritionist, clerks, orderlies and housekeepers.
