Patient File
A patient file is created for everyone receiving care and services at Hôpital Montfort. The Disclosure Service deals with all requests for information contained in these files.
To consult your patient file, obtain a proof of birth, of stay or of death, please go to the hospital’s Release of Health Information Office (00D135). An employee will be pleased to help you.
You must present proof of identity (a driver’s licence or passport, for example) and fill out a Request for Disclosure form.
Please note that fees are applicable.
