Social Services for Acute Care
Montfort offers social services to its patients when acute care is required. Experienced social workers are part of a multidisciplinary team that helps hospitalized patients as well as patients in the Emergency Department. This team is specialized in evaluation and intervention when there are domestic, social or economic factors that have an impact on a patient’s health.
When a patient is unable to return home, the social workers can help that person make appropriate living arrangements.
